Log in to your account and go to "Antivirus" → "Control Panel":

Make sure your insurance policy is active. If it is inactive, you must reactivate it according to the instructions.
If your insurance policy is active, you can click the "File a Claim" button:

Describe your problem, attach files if necessary, and click "Submit Claim":

You can view your claim by clicking the "View Claim" button:

You can view all your claims in the "Support" tab:
